I am having issues whereby blocks are exploded to individual; entities when I insert them.
It happens with any block and any of the block insertion methods. If insert a block say using the file explorer I may find that the block is exploded when inserted. However if I use the block list it may be inserted as a block. There is no consistency in terms of whether it is imported as a block or not.
Is there a setting that may cause this behaviour?

Please check your preference:
Edit > Application Preferences > Graphics View > Behaviour > Edit block reference in-place
If the option is ticked, it could be that you have a habit of editing blocks inline and then saving the file without closing the block that is being edited, leaving it exploded. This is just a wild guess.
To look into your problem, I'd need your drawing file and the file of the drawing you are trying to insert as block.
