I recently adopt Qcad pro,
I use fractional units for cabinets drawings (our standard in Canada). I need to put dimensions as ex. 48" in my drawing. Yhen the question is : How can I put the suffix " to represent inch units ?

below "Drawing Preferences" you could change the Dim Label Format to "Architectural" - maybe that works better for you ...
This said - common praxis would be to use "Fraction" as Label Format and declare on the drawing that the unit inch is used.

Unfortunatly, I can't used Architectural Units for Cabinet drawing.
Instead of ex. 4'-0" I really need to write 48".
Maybe there is no solution ?

I don't know if there is a solution for you but we are not yet on the end of the road ...
Using plain 48 and declare that the drawing is in inch doesn't work for you? Why not?
What QCAD version do you use? Do you familiar with the use of the QCAD Selection Filter and Property Editor?

Would changing the format to Fractional give you what you're after ??

There is no way to do that persistent.
Basically because drawings are essentially dimensionless.
In the Property Editor enter <>" literal for the Label field of a selected dimension, <> is the placeholder for the measured size.
One can do that when creating dimensions too but such is not retained as option, the same with prefixes.
One can also select all dimensions afterwards and edit the label field as per above in one go.
