I use Big Sur 11.5.1 on a Mac with an M1. I run QCAD Pro.
I have an existing dimensioned drawing that I would like to scale up and down. Is there a way to select one measurement on the existing drawing and change it and have all other dimensions scale equally?

What you need to do is scale everything with a given or calculated factor.
First about the source:
A dimension is a rounded value and the length property of a line in the Property Editor is also a rounded value.
Using a rounded value in a factor is only about right.
It will be correct if the value has fewer significant digits than the displayed precision.

A right click on the Command Line also lets you create a variable with the exact value.
The variable stays valid during whole the current QCAD session.
What current size you want to use is of your choice.
We can then scale all things with a factor based on the required size divided by the current size.
Menu Select .. Select All (TA)
Menu Modify .. Scale (SZ)
Indicate the scaling origin.
In the dialog opt for Delete original; Proportional; Not using current layer and attributes
In the Factor x field enter the equation based on the two sizes or the required divided by the measured variable.
