While once the Nesting feature was decent if slow, now it does nothing useful at all. It will now not nest even trivial cases, often not fitting more than 30% of the pieces when 200% will fit.

Sounds like the Qcad configuration is somehow damaged. If you can't figure what could be set wrongly try to reset Qcad to its defaults.

Hi,
Your Nesting preferences look fine.
They probably include 'Disable AppNap' because you are working on a MacBook, on Windows this preference does not exist.
I remark one Nesting Option in the option Toolbar where I have some reservations about.
Tolerance = 100
Even with a trivial rectangle to nest in a larger rectangle I get no solutions with that amount of tolerance.
The default value 0.001 works fine in most common cases.
We could help you more efficiently with an example drawing file.
Rather weird that a huge tolerance give less good results.
And true, a full configuration reset would revert to the CAM nesting default tolerance ...
... But you will loose all other QCAD(/CAM) customization's, user preferences and persisting options in doing that.
